What Is SOIUI?

Combining Superovulation (SO) which uses hormonal medication to stimulate follicles or eggs to grow and ovulate, and Intra-Uterine Insemination (IUI) in which specially prepared enhanced sperms are inseminated into the uterus, SOIUI helps increase the chances of conception.

In SOIUI, hormonal medications are administered to stimulate the ovaries to produce 1-3 follicles to mature and ovulate thereby increasing the number of eggs available for fertilisation. This is in contrast to regular IUI, which may not involve any hormonal stimulation and typically relies on the natural ovulation cycle.

Who Is Eligible for SOIUI Treatment?

Superovulation Intra-Uterine Insemination (SOIUI) is recommended for couples who are experiencing mild male infertility, mild endometriosis, cervical factor infertility, unexplained infertility and after several failed clomiphene or letrazole (oral ovulation medication) cycles. The female patient should have patent fallopian tubes and the husband should have a supply of reasonably healthy sperm.

Generally, SOIUI is recommended for women under the age of 35 with no other significant fertility issues or women over 35 years old that cannot afford IVF (In vitro fertilization) treatment. As women age, the quality and quantity of their eggs decrease. Additionally, both partners should be in good overall health, with no untreated medical conditions that could impact fertility, such as thyroid disorders, uncontrolled diabetes or autoimmune diseases.

At the same time, some medical conditions may make a couple unsuitable for SOIUI treatment, including:

  • Severe male factor infertility, such as very low sperm count or poor sperm quality
  • Blocked or damaged fallopian tubes in the female partner
  • Severe endometriosis or advanced stages of the condition requiring surgery
  • Uterine fibroids that distort the uterine cavity

How Should I Prepare for an SOIUI Cycle?

Before beginning an SOIUI cycle, couples should undergo an initial consultation with a fertility gynaecologist. This will typically include:

  • Fertility Testing – Comprehensive evaluations such as blood tests to assess hormone levels, ovarian reserve and genetic screening. A hysterosalpingogram (HSG) may also be performed to determine if the fallopian tubes are patent.
  • Discussion of Treatment Protocols – The doctor will explain the SOIUI process, including the medications that will be used to stimulate the ovaries and the timing of insemination.

How Does the SOIUI Procedure Work?

Our Singapore fertility doctor will prescribe fertility drugs (oral or injections) to stimulate the growth of ovarian follicles. Serial ultrasound scans will help determine the number and size of eggs within the ovary. The insemination of the sperms into the uterus is performed just prior to ovulation. On the day of the insemination (IUI), the male partner will be required to produce fresh semen at the fertility centre. The enhanced sperms are carefully prepared and inseminated into the uterus via a fine catheter.

What Can I Expect After Undergoing the SOIUI Procedure?

After the insemination, the patient will be given hormonal medications to improve the chances of implantation and pregnancy. The patient is advised to return to our fertility clinic 14 to 17 days after the IUI for a pregnancy test.

What Are the Risks Associated With SOIUI?

While it is a safe and established procedure, SOIUI still has risks that patients should be aware of, such as:

  • Mild Discomfort – The procedure may cause mild cramping or discomfort during and after the catheter insertion.
  • Spotting – Some women may experience light vaginal bleeding, known as spotting, following the procedure.
  • Infection – There is a very low risk of infection due to the introduction of bacteria during the catheter insertion.
  • Multiple Pregnancy – The likelihood of conceiving twins or higher-order multiples increases if the woman ovulates 2-3 eggs simultaneously.
  • Ovarian Hyperstimulation Syndrome (OHSS) – This condition can occur if the ovaries are overstimulated because of the fertility medications, leading to swollen ovaries and abdominal discomfort.

Success Rates

The success rate of a SOIUI procedure is about 14 to 15% per attempt, and a large part depends on the woman’s age, the man’s sperm quality and immaculate timing of the IUI procedure.

Female Factors

  • Age – Age plays a crucial role in determining success rates. Women under 35 typically have higher success rates which gradually declines with age.
  • Ovarian Function – The health and function of the ovaries, including the number of viable eggs, are important. Healthy women with good ovarian reserves have better chances of success.
  • Fallopian Tube Condition – Patent and functioning fallopian tubes are essential for the egg and sperm to fertilize.
  • Endometriosis – The presence of endometriosis can negatively impact fertility and reduce the chances of conception.
  • Uterine Abnormalities – Any structural issues with the uterus, such as fibroids, polyps in the uterine cavity or uterine deformities can hinder implantation and reduce success rates.

Male Factors

  • Sperm Quality – Sperm quality is determined by its shape (morphology), movement (motility), viability and its DNA integrity.
  • Sperm Count – The number of sperm present, with higher counts increasing the likelihood of successful fertilisation.

Timing of Insemination

The timing of the IUI procedure is crucial. The insemination must be scheduled to coincide with ovulation, maximising the chances of sperm fertilizing the egg. Proper monitoring through urine ovulation tests and ultrasounds can help to determine the optimal time for insemination.

What Can Be Done to Increase the Success Rate of SOIUI?

Increasing the success rate of the SOIUI procedure involves a combination of medical, lifestyle and timing considerations. These include:

  • Precise Ovulation Tracking – Accurate ovulation tracking is crucial for timing of the insemination. Utilising urine ovulation test kits and ultrasound monitoring can help pinpoint the optimal time for the IUI procedure.
  • Sperm Preparation – Proper preparation of the sperm sample enhances the chances of successful fertilisation. This involves "washing" the sperm to separate normal motile viable sperms from the original seminal fluid for insemination which improves sperm quality and success rates.
  • Multiple Cycles – Engaging in multiple SOIUI cycles can improve overall success rates. While the success rate per cycle is around 14 to 15%, cumulative success increases with each attempt, as couples may improve their response to treatment so that 3 SOIUI cycles has an overall success rate of 25% which is equal to 1 IVF treatment but at a much lower cost.
  • Healthy Lifestyle – Adopting a healthy lifestyle can positively impact fertility. A balanced diet and regular exercise can improve overall health. Avoiding smoking and excessive alcohol consumption can also enhance reproductive health for both partners.
  • Weight Management – Maintaining a healthy weight is essential for optimal fertility. Underweight and overweight individuals may experience hormonal imbalances that affect ovulation and sperm quality.
  • Stress Management – High stress levels can negatively impact fertility and overall health. Engaging in stress management measures can help create a more conducive environment for conception and improve the chances of success with SOIUI.
